Our team links care across multiple sites for patients, families and colleagues in an effort to restore, maintain and promote health and reduce the effects of disease and disability.

Home Care Patient and Family Advisory Council

A patient and family advisory council (PFAC) is an organization of current and former patients, family members and caregivers that works together to advance best practices. Learn more about the importance of a PFAC.  

Our Home Care PFAC collaborates with home care leadership and staff to enhance the patient and family experience. The group meets monthly to discuss new programs, share educational materials and stay informed about our work.
